Inicialmente anunciado como um documentário sobre a indústria farmacêutica, várias empresas desta área prontamente tomaram medidas para evitar o acesso de Michael Moore a quaisquer informações comprometedoras e aconselharam os seus funcionários a não dialogar com o cineasta. Irreverente e incisivo como habitualmente, Moore surpreendeu de novo, produzindo um documentário sobre o lucrativo sistema de saúde norte-americano.
